My Motorola DCT-3400 STB gives off a high pitched squeal on or off, sort of like a dog whistle, any ideas why it’s doing this?

Could be either the fan or the disk drive motor. Both stay powered up when the STB is off.
You may have to get it replaced.

Oops, well it turns out it was not the cable box. When I removed my switcher I left the AC adapter plugged in and for some reason that's what was causing it. I unplugged it and no squeal.

Glad you found the problem. At least AC adapters don't cost much. Nothing if your under warranty still.
