I am chief engineer a North Central University and we run our own cable system. This summer we put five HDTV channels on the system using Drake equipment that simply converts the over the air 8VSB to a cable channel.
This worked fine except about half the TVs we try will not pick up the HD channels.
It seems some tuners (Samsung and LG for sure) will only look for 8VSB on off-air channels and only QAM when auto programming cable channels.
Any one else out there have any experience with this.

For digital (HD):
8VSB is ATSC (OTA) antenna.
QAM is for digital cable.

Internal tuners in TV's know nothing more.
