Audio/Video out of sync?

Hey Everyone,
I was just wondering what could cause the video and audio to go out of sync every now and then. I have a Rogers HD box, and a sony Bravia LCD TV. I was just wondering, that sometimes some random channels are out of sync. Would this be caused by the TV or the box? Also, what could be done to fix the problem when it happens?

It's usually caused by a problem with the affiliate, especially if it's limited to one channel. If it's all channels, then it's usually the STB and a cold reboot fixes it (unplug, replug). See:
