Had something similiar happen, I didn't have a spare receiver. When I called Dish they wanted $75 for a new card. It was an old receiver so I just went down to the A/V store and bought a new basic receiver for $125 (a 301). Called Dish and told them the new serial (?) number that came up on the screen and they enabled the system.

I don't know the exact use of the card except it identifies the box and you as a customer. You could call Dish/DTV and ask them if the card out of an old receiver would work. I also think there are different types or revisions of cards.

Give 'em a call they'll know the answers
